Chronic Disease Management in Boca Raton

Chronic Disease Management

Specialized, proactive care for hypertension, diabetes, heart disease, and other ongoing conditions.

Chronic disease does not respond well to the 10-minute appointment. It responds to close monitoring, a physician who understands your trajectory, and the time to adjust medications and lifestyle as your condition evolves. That is exactly what concierge medicine makes possible, and why our members with chronic conditions consistently achieve better control.

A More Attentive Model

How PBCM Chronic Care Is Different

Per the CDC, six in ten U.S. adults live with a chronic disease, and four in ten live with two or more. In traditional primary care, complex chronic patients are squeezed into 15-minute slots every three to six months, which is nowhere near enough time to genuinely optimize medications, lifestyle, and monitoring.

At PBCM, chronic disease management looks different:

01

Extended 30–60 minute visits, as often as your condition requires

02

Between-visit access: text, call, or secure message your physician the day something changes

03

Real medication optimization, not just refilling the same prescriptions year after year

04

Lab and biometric trend analysis so we catch drift before it becomes decompensation

05

Tight specialist coordination when cardiology, endocrinology, or nephrology input is needed

06

Lifestyle integration: nutrition, exercise, sleep, and stress treated as medicine, not an afterthought

Close Monitoring Over Time

A Typical Ongoing Care Plan

Your chronic-care plan is built around your specific conditions and goals. A typical quarterly cycle might include:

Pre-Visit Review

Review of home readings, including blood pressure, glucose, weight, lab trends, and recent symptoms.

Physician Visit

An in-person or telemedicine visit to review changes and adjust medications.

Follow-Up Labs

A follow-up lab panel calibrated to what we're actively monitoring.

Quick Check-Ins

Quick check-ins between visits as needed, with no additional copay and no appointment-booking gauntlet.

Trusted Chronic Disease Resources

Chronic care should be proactive, coordinated, and evidence-informed.

PBCM chronic disease management is built around close monitoring, medication optimization, prevention, and the trusted guidance of your physician.

Frequently Asked Questions

Chronic Disease Management FAQs

Can you replace my cardiologist or endocrinologist?

No, and we wouldn't try to. Dr. Aziz-Siddiqui co-manages complex conditions alongside specialists, handles the day-to-day optimization, and makes sure the specialist's recommendations are implemented and tracked in your overall plan.

Do you prescribe GLP-1 medications, such as semaglutide or tirzepatide?

Yes, when clinically appropriate, typically as part of our weight management and metabolic health program for patients who meet the criteria. All prescribing is done under close physician supervision with appropriate monitoring.

How often will I be seen?

It depends on your condition and stability. Well-controlled hypertension might mean every 3–6 months; newly diagnosed diabetes may mean every 4–6 weeks until we have you stabilized. Your schedule is personalized, not mechanical.

What if my condition suddenly worsens?

Reach out directly by text, call, or message. Urgent concerns are typically addressed within hours, not days.
