Our office is at 9970 Central Park Blvd N, Suite 303, Boca Raton, FL 33428, in West Boca Raton. For most Parkland residents, it is a short drive east or northeast toward West Boca, typically around 10 to 15 minutes depending on traffic and your starting neighborhood.

Many members combine in-office visits with telemedicine for routine follow-ups, which keeps drive time out of your week without giving up the depth of an in-person relationship.

Office hours are Monday to Friday, 9:00 AM to 5:00 PM. After-hours availability is for urgent and emergency concerns by phone, text, or email.

Do you see both spouses or partners as members?

Yes. Couples and family adults are common in the practice. Each adult enrolls as their own member, with their own physician relationship and care plan. Coordination across a household is straightforward when both partners are members.

Can I see Dr. Aziz-Siddiqui by telemedicine if I live in Parkland?

Yes. Telemedicine is included in your membership and is well-suited to follow-up visits, lab review, medication adjustments, many sick visits, and clinical questions. All visits run on a HIPAA-compliant platform.
